You are confusing mental gender with biological sex. Being born with a Y chromosome and believing you're really female doesn't change the fact that you have a penis. But it may affect both how you behave and how you identify, which may or may not conform to societal gender norms. Which, despite being illusory ideas and not something as concrete as a certain type of chromosome or form of reproductive organ, do matter quite a bit. Just as the fact that social attitudes can make an intergenerational sexual encounter harmful despite the encounter not involving intrinsic harm also matters - enough that it can't just be ignored. There is absolutely nothing simple about gender and it is the oversimplification of gender that leads to people being utterly confused, because they don't fit into one or the other pigeonhole.
