You are right.

This reminds me of the global trend towards "medical tourism" where first world patients travel to third world nations for care, where they go to private hospitals (staffed by locals as well as first world expats) and pay directly, but at a lower cost, or shorter waiting times, than they do in their own countries; where either socialized care results in long waiting times, or private insurance makes it too expensive.

In that case the barrier is clearly red tape in the first world countries, and certainly not the quality of infrastructure or doctors or the sums of money in the industry.

But of course, the first world poor are not mobile enough to do this, and they get stuck with the worst of their own country's systems.
