You're looking at lower calf to ankle(the rest is all under the dress) which is not commonly thinner than the arm in general, but it's possible for a girl to be proportioned like that. Also, consider the light source; that shadow enveloping the underside of her leg. Could be a trick of light which is even possible in a photo(or RL even).

The proportions of her face are very realistic, this is not an imcompetent artist. :)
