Was he banned for good and did he ever ask to be reinstated?
He was banned for good. I don't think he ever asked to be reinstated.
If not then was he ever suspected of returning under another username?

Do many others do this and have any been detected? Just wondering ...
It's actually (and regrettably) very common. Some have even done this as many times as Jason and Freddy. However they have so far been detected either by technical means or by repeating their offensive patterns.
