To my mind, acting/not acting upon it, and suffering distress are like worlds apart, or not at all in any way intrinsically related to each other.
Of course this is an observation of mine regarding the APA definitions, and further, these definitions seem to be limited in scope.
But then, one should not expect too much/a great deal from psyvhiatrists/psychologist.
