IMV, the essential difference is that C is irreversible as a physical act, whereas R is, in most cases a temporary condition (of course apart from the iatrogenic factors, etc).
What is lost is lost for good (bad?) whereas R in many cases can be gotten over with (but remains a horrific act TBS).
