In Gogol's Dead Souls two ladies, The Simply Pleasant Lady and The Lady Who Is Pleasant In All Respects, have a lengthy dialogue, and Gogol has either accidentally or on purpose lost count of which lady is saying what to the other lady - just because he does not use what-are-they-called "A said to B" clarifications in this dialogue.
It is rare to see such a confusion in novels.
