The difference between a marriage that lasts and one that doesn't often depends on whether or not you are willing to stay true to the oath you swore on your wedding day REGARDLESS of your personal feelings.
About a year ago, I COULD have easily cheated on my wife with a 16yo girl who was nuts about me (and had the lack of morals to not care that I'm married). She was an incredibly hot young lady and there was certainly a temptation (hey, I'm married not dead!), but I never once even seriously considered the possibility of cheating on my wife. Why? Because I took vows and I intend to stick with them. Other women and other loves may come and go, but having a companion to share your life with is a LOT more important than any temporary happiness could ever be. I made my choice, just as you made yours. We could've chosen to live a life where we "follow our hearts" wherever they lead us from day to day, but we didn't. Instead, we choose to forsake that existance in favor of never having to be alone again - never having to risk being rejected or misunderstood or having to go through all those stupid ass dating rituals!
In other words, my advice is this: You made your decision already on the day you married your wife. There shouldn't be anything for you to think about because you've already decided this!
Now, do I think you should tell your wife or something? Fuck no, don't be stupid! If my wife cheated on me, I'd damn sure rather not know about it as long as she realized it was a mistake and came back to me (for good).
Tell your 18yo that you made a mistake. You're a married man and so it is simply not possible to be with her. Go back to your wife and forget that this ever happened.
If, however, this stems from not being happy with your wife in the first place.... well, that's a whole different story and negates everything I've written :)
