There is indeed a deep psychological meaning behind this.
The toddler handed over the cap - the female portion of the unit, whilst retaining the marker itself, the male portion.
Essentially, she was initiating a gender swap.
In unspoken words she was saying, "How would YOU like being a little girl stuck on the floor of a doctor's waiting room with nothing to do but scribble, whilst waiting to have your bits poked at by a doctor?"
Well, its a theory. Not a good theory, but still a theory.
